Providing diabetes care in the community

Our community clinics, led by consultants, take place regularly in Clacton, Colchester and Harwich.
Our team is made up of nurse consultants, diabetic specialist nurses, dietitians, Consultant diabetologists, podiatrists, Health Care Assistant and administrators.

We aim to educate and support our patients in the management of their diabetes to ensure they continue to live a happy, healthy life.

We will:
• Work with patients to achieve the best possible outcomes.
• Provide individual diabetes care, adapted to suit each patients’ needs.
• Offer a variety of different education sessions to patients’ post-diagnosis.
• Empower those living with diabetes by giving them the confidence to self-manage their condition

Other specialist clinics, such as insulin pump, exercise and insulin, HCA clinics, take place in a variety of locations on a regular basis.

We also work collaboratively with the local ‘Therapy for You’ service to provide easier access for people living with diabetes to get support with managing their psychological wellbeing. These clinics are currently held at our Colchester clinic location.

When you are invited to make an appointment, the administrator will make you aware of the days and times of each of the clinics and your confirmation letter will include all the details that you will need to find the location.
Please note that these clinics do not offer a walk-in service.

What to bring to your clinic appointment:
• Your blood glucose monitor (BGM) or your BGM diary
• Make a note of your blood pressure measurements in the 2-3 days leading up to your appointment
• A list of your current medications
• Any questions that you would like to ask our team

Assistance with travel costs to attend your appointment
Please call the Department of Health customer enquiry line on 0300 330 1343. This service is available if you are on a low income and may help you claim back travel costs for your appointment to our Service.
